Breast implants can either be placed either above the pectoralis muscle
(subglandular) or below the pectoralis muscle (subpectoral) in each of
your breasts. Each placement has its pros and cons. Placing implants over
the muscle usually results in decreased pain and recovery time because
the muscles were not disrupted. However, this positioning can make the
implants look less natural, have a higher incidence of capsular contracture
and in those women without ample breast tissue coverage can have higher
risk of visible rippling and wrinkling. Placing implants under the muscle
usually looks more natural, improves ease of reading mammograms, and decreases
risk of rippling and wrinkling in thin patients. The downside of this
placement is a longer or more painful recovery and there is the possibility
of implant movement or distortion when the pectoralis muscles are flexed.
